Updated CICM Fellowship study notes

Three of the CIC Fellowship study notes have been updated:

1. Biostatistics (in the Statistics folder, under Core ICU) – New information on metanalysis heterogeneity tests.
2. Ventilator associated pneumonia (in the Ventilation folder, under Core ICU) – New metanalysis in support of selective oral decontamination for the prevention of VAP.
3. Imporatant trials – A new file, summarising key critical care trials in short, point form. Found in the CICM Notes folder.
