Chloride: the devil’s anion?
Debate continues over the potential evils of the anion that is rotting kidneys, killing patients with SIRS and generally giving salt a bad name.

New Zealand’s crit care research guru Paul Young is currently running the SPLIT trial, which is looking at whether N/Saline causes more acute kidney injury when compared to Plasma-Lyte 148® – see more here and at SMACC Chicago.
The evidence base on this topic is currently mixed, with a Meta-analysis of high- versus low-chloride content in perioperative and critical care fluid resuscitation showing no mortality benefit, however, an association between intravenous chloride load during resuscitation and in-hospital mortality has been demonstrated among patients with SIRS.
